  1. The Emergency Sagot ng PNB Promo (the “Promo”) shall run from September 10, 2015 to June 30, 2016 (the “Promo Period”).
  2. The Promo is open to individuals who are customers of Philippine National Bank (“PNB”) who must meet the following qualifications (the “Qualified Customer”):
    1. received at least three (3) remittances with a minimum amount of US$300.00 or Php 12,000.00 each from any PNB overseas branch/remittance office/agents and bank tie–ups in the following countries during the Promo Period:
      1. United States of America
      2. Canada
      3. Europe
      4. Singapore (remittances coursed through bank tie-ups are not qualified in the promo)
      5. Japan
      6. Hong Kong
      7. Middle East
      8. Guam (remittances coursed through agents of PNB in Guam are not qualified in the Promo);
    2. registered via SMS to join the Promo within the Promo Period; and
    3. are not prohibited under applicable Gifts, Anti-Bribery and Corruption laws, regulations and policies from participating in or qualifying for this Promo.
    4. other qualifications as set forth below.
  3. The Qualified Customer shall be entitled to a free Healthy Ka Pinoy Emergency Expense Card (the “HKP Card”) with the following insurance package:

  4. The Qualified Customer will only be entitled to one (1) HKP Card for a period of six months. No double life insurance on the same coverage period shall be allowed. However, the Qualified Customer may redeem up to two (2) HKP Cards equivalent to six (6) remittances registered or equivalent to one (1) year worth of insurance coverage.
  5. The Qualified Customer may nominate himself/herself for the HKP Card or may nominate another person to avail of the HKP Card; provided, that the nominee’s age must not be less than five (5) years old but not more than sixty-five (65) years old at the time of nomination and for the duration of the validity of the HKP Card.
  6. To join the Promo, the Qualified Customer must register via SMS by providing his/her mobile number and his/her remittance transaction reference number, and sending the message below to 3456:

  7. The mobile number used by the Qualified Customer upon his/her registration will serve as his/her joining ID.
  8. Each remittance transaction reference number must be registered via SMS for it to be counted against the Qualified Customer’s joining ID.
  9. A confirmation message will be sent to the Qualified Customer within forty-eight (48) hours once the registration was successful.
  10. In case of a Global Filipino Cardholder with one (1) remittance credit for the past six (6) months, he/she is qualified to instantly redeem a maximum of one (1) HKP Card by registering his/her Global Filipino Card number and sending the message below to 3456:

  11. The Qualified Customer/ GFC Cardholder will receive a congratulatory SMS message on the date shown below:

  12. Upon receipt of the congratulatory SMS message, the Qualified Customer/ GFC Holder must go to any PNB branch in the Philippines on or before September 15, 2016 or within sixty (60) days from date of receipt of the congratulatory SMS message, to claim his/her HKP Card.
  13. To claim his/her HKP Card the Qualified Customer/ GFC Holder must:
    1. Present the SMS Congratulatory message to the PNB branch representative.
    2. Submit a photocopy of valid photo-bearing and signature-bearing ID to the branch representative.
    3. Accomplish and submit the Healthy Ka Pinoy Client Information Sheet to the branch representative.
  14. Upon submission of the Healthy Ka Pinoy Client Information Sheet, the Qualified Customer/ GFC Holder must allow the PNB branch representative to delete the congratulatory SMS message.
  15. The Qualified Customer/ GFC Holder must personally claim the HKP Card. In the event that the Qualified Customer/ GFC Holder is unavailable, he/she can send an authorized representative to receive the HKP Card, provided that the authorized representative will submit an authorization letter from the Qualified Customer/GFC Holder, with all the requirements specified at Item No. 12, and must present said authorized representative’s valid photo-bearing ID.
  16. 1The HKP Card shall be activated within two (2) weeks from receipt by Qualified Customer/ GFC Holder of the HKP Card (the “HKP Cardholder”).
  17. In the event that the HKP Cardholder should need to claim a covered insurance benefit prior to the card activation, the HKP Cardholder will need to make an out-of-pocket payment first then claim reimbursement of the amount he/she paid for the emergency or accidental event that occurred during the period that the HKP Card was not yet active.
  18. The HKP Card cannot be exchanged for cash.
  19. PNB shall not be obliged to recognize or re-send lost, deleted or misplaced congratulatory SMS message.
  20. PNB shall not be responsible for any matter related to the service/products provided by the insurance provider. Any information, query, complaint, or concern pertaining to the benefits, procedures, claims, inclusions, exclusions, obligations, or liabilities of the products or services provided by the insurance provider shall be referred directly to said insurance provider.
